Notice of Privacy Practices (HIPAA)

Bream Optometry is committed to protecting the privacy of your protected health information (PHI) in accordance with the federal Health Insurance Portability and Accountability Act (HIPAA). We use and disclose your health information only as permitted by law — primarily for treatment, payment, and health care operations.

You have the right to inspect and request copies of your records, request amendments, request restrictions on certain uses and disclosures, request confidential communications, and receive an accounting of certain disclosures. California Confidentiality of Medical Information Act (CMIA)

In addition to HIPAA, your medical information is protected under the California Confidentiality of Medical Information Act (Civil Code §56 et seq.). We will not disclose your medical information without your written authorization except where permitted or required by California law.

Your California Privacy Rights (CCPA/CPRA)

Under the California Consumer Privacy Act, as amended by the California Privacy Rights Act, California residents have rights regarding personal information that is not otherwise exempt as medical information. These may include the right to know what personal information we collect, the right to request deletion, the right to correct inaccurate information, and the right to opt out of the sale or sharing of personal information.

Your Prescription Rights

Following a contact lens or eyeglass examination, you are entitled to a copy of your prescription in accordance with the federal Eyeglass Rule, the Contact Lens Rule, and California Business and Professions Code provisions governing optometry. You are not required to purchase eyewear or contact lenses from any particular provider.

Website Use & Appointment Booking

Online booking is handled through a third-party scheduling provider. Information you submit through that service is governed by the provider's own privacy and security practices. This website is provided for general informational purposes and does not constitute medical advice. Always consult a qualified eye-care professional regarding your specific condition.
